As a Change Mental Health Practitioner, you will work directly with service users requiring immediate or ongoing emotional, practical, or vocational support to achieve the best possible outcomes. You will develop and review personal plans in collaboration with service users, with ultimate oversight by the Senior Project Worker. You will work closely with team members to meet objectives set by your line manager. The team has a clear management structure to support and guide your practice. You will contribute to creating an environment where individuals can relax, rebuild social skills, and improve their quality of life.
