The Imperial College Renal and Transplant Centre opened at the Hammersmith Hospital in November 2005 and has become the largest facility of its kind in Europe.
We have 4 inpatient wards totaling 71 beds (12 HDU) and a programmed investigation unit offering a ward attender/day case service of 18 beds.
The service also has an acute haemodialysis unit, 9 satellite dialysis units, peritoneal dialysis and home haemodialysis teams; nurse led chronic kidney disease team, Live donor team and 2 outpatient clinics looking after over 3,100 patients on renal replacement therapy.
The Renal Clinic at Hammersmith Hospital provides ongoing outpatient care for our transplant, Vasculitis, Lupus patients. We have combined cardiac, rheumatology, ENT, Eye clinic also.
We are looking for an experienced renal nurse to become part of this highly skilled team. You will oversee the delivery of high quality, safe, effective care focused on improving outcomes and experiences for patients, families and carers.

Main duties of the job
1. Carry out planned care for a group of patients without direct supervision.
2. Ensure the assessment, planning, implementation and evaluation of evidence based, individualised patient care as appropriate within an outpatient setting.
3. To be responsible for reception and appointment functions within the department, ensuring that systems are in place in order that patients are seen in a timely manner.
4. Working with identified Consultant Nephrologists to ensure that results review/audit and admission planning takes place.
5. Assist and advise staff, both registered and unregistered in the delivery of patient care.
6. Ensure all staff maintain accurate clinical observations of the patient and act accordingly
7. Undertake nursing procedures
8. Have responsibility for the correct administration and custody of medicines according to Trust policy
9. Deal with concerns raised by patients and visitors in a proactive manner and take remedial action as necessary.
10. Act as the patient’s advocate
11. Oversee effective performance and quality of non-clinical and environmental services that impact on patient care, i.e. portering/domestic services.
