Position: Clinical Nurse Assessment Specialist
Location: Nationwide role covering our existing and new portfolio of homes. The ability to drive and access to a car is essential.
Contract type: Full time, 40 hours per week (Monday to Friday)

Join us as our new Clinical Nurse Assessment Specialist at Exemplar Health Care.
Reporting to the Senior Clinical Assessment Specialist, you’ll work flexibly as part of a motivated Clinical Assessment Team, supporting our specialist care homes to respond to referrals.
We have an ambitious growth plan to expand our portfolio of homes in the coming years. You will help ensure every referral is managed to the highest standard so our customers receive an exemplary experience.

About the role
Day-to-day responsibilities include:
* Support care homes to complete clinical pre-admission assessments, traveling to different care settings nationwide within the allocated working day.
* Produce individual cost per care assessments showing the proposed specialist care package and justification for interventions and costs. This can be done in the office or remotely from home.
* Coach Clinical Nurse Managers and Home Managers in assessment and costing processes and assist with delivering training.
* Support processing of referrals at the request of the Senior Clinical Assessment Specialist, working with the Referrals and Occupancy Management Team.
* Uphold the company vision and drive Exemplar Health Care’s strategy.
* Support reassessment of existing service users where required.
* Build relationships with new and potential referrers.
* Attend conferences, events and networks to raise Exemplar Health Care’s profile as required.
About you

* Excellent verbal and written communication with staff and stakeholders.
* A passion for working with individuals with complex health needs.
* A desire to work in a fast-paced, responsive team, developing a systematic approach to referral management and ensuring high customer experience.
* Flexibility to work independently and as part of a team, with good organisation and self-motivation.
* Willingness to travel to portfolio homes.
* Confidence representing Exemplar Health Care at internal, commissioner and contract meetings.
* First-level nursing registration with a valid NMC PIN. We welcome Registered Nurses from Adult, Mental Health and Learning Disability backgrounds.
