About the Role
We are looking to recruit two dynamic and experienced Social Workers for a community mental health service in Weymouth and Portland. The team works in an integrated community setting alongside health colleagues to support people with significant mental health needs and help them transition into adulthood. It is a supportive, nurturing service that promotes a recovery model and person‑centred practice.
Key Responsibilities
* Undertake strengths‑based assessments, support planning and reviews.
* Conduct Mental Capacity Assessments.
* Work with people subject to the Mental Health Act, completing S117 assessments and reviews.
* Co‑locate with health colleagues – Community Mental Health Nurses, OT, Psychologist, Psychiatrist – to provide services for adults with Serious Mental Illness or Autistic Spectrum Condition.
* Collaborate with other health and social care professionals.
* Identify carers’ needs and support their assessments.
* Take leadership roles and support service development.
Qualifications and Experience
* Passion and commitment for working with people with significant mental health problems and promoting their recovery.
* Excellent communication skills and ability to work in a fast‑paced environment.
* Flexible approach and ability to work closely with a range of professionals.
* Confidence in the legal basis for practice and a good understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998 and Equality Act 2010.
* Degree in Social Work or equivalent.
* Ability to fulfil the travel requirements of the post.
