Following the successful RCOT pilot, you will help individuals manage common mental health conditions to prevent long-term sickness, making a real difference in their recovery journey
This role provides specialist physical health support within a vocational rehabilitation framework, working closely with primary care and community services supporting patients in our ground‑breaking Trailblazer 'Life Re-worked' Programme.
Responsibilities
* The post holder will be a member of Merton Vocational Rehabilitation team as part of the Trailblazer pilot to help residents return to work by overcoming mental health barriers.
* Responsible for undertaking initial and comprehensive assessment of health and social care needs alone or with colleagues and feedback to Multi-Disciplinary Team.
* They will have care co‑ordination and co‑worker responsibility for the overall care of an identified group of service users, being part of a service that is committed to recovery and social inclusion.
* Provide supervision to an identified group of staff and students on placement with the team.
* They will develop and maintain effective communication strategies between Mental Health Services and General Practice Teams to improve care pathways.
* The post holder will be required to work within CLCH policies and procedures, professional and legal framework upholding the values and ethos of the organisation.
* To have a clear understanding and practice within framework of one's Code of Professional Conduct, Adult and Child protection protocols, Mental Health and Capacity Acts.
Qualifications
* BSc Degree or equivalent in Physiotherapy
* Evidence of continuous professional and personal development
* Registration with the Health and Care Professions Council (HCPC)
* Member of a specialist group
* CSP membership
Experience
* Experience of working in an outpatient department
* Experience of working within NHS environment
* Experience of multidisciplinary and inter‑agency working
* Experience of working with a wide range of patient conditions
* Research project carried out at undergraduate level
* Experience of planning and delivering clinical supervision / mentoring to peers and less experienced physiotherapists
* Experience of assisting in projects within team
* Experience at Band 6 level
Skills
* Knowledge of Clinical Governance and its influence on Service Development
* Knowledge of recent research supporting or negating physiotherapy management of neuromusculoskeletal conditions
* Knowledge of equal opportunities
* Effective oral, listening and written communication skills in order to successfully assess and treat patients and manage their outcomes
* Effective interpersonal skills to liaise with all professionals to enhance patient care pathways
* Listening skills and interpersonal skills to discuss patient care with colleaguesEffective physiotherapy manual skills to assess and treat patients with neuromusculoskeletal conditions.
* To work at or towards M level standard towards a full Masters qualification
* Knowledge of NICE guidelines and the NSF's
* Presentation skills to lead and teach in-service training sessions
Key Attributes
* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
