Main duties of the job: This is a great opportunity for those who feel they no longer have the time to give basic holistic care to their patients. The team works to support and empower each other to deliver the highest standard of person-centred care.
The service covers from 7am to 7pm, 7 days a week. You will need to work a mixture of the following shift times to cover the needs of the service: 7am to 3pm, 8.30am to 4.30pm and 11pm to 7pm.
You will work as a member of a neighbourhood nursing team that delivers person-centred integrated care in the community. The aim of the service is to support people to remain as independent at home as long as possible, proactively facilitating self-care.
Care is focused on improving self-reliance and empowerment; seeing patients as a whole and supporting as required their health, social and psychological needs.
Job Responsibilities
* Ability to form professional, trusting and reliable relationships with service users and their families.
* Good inter-personal and communication skills, including ability to deal with sensitive service users issues and concerns.
* Provide and receive routine information either verbally or in writing and understanding of any potential barriers.
* Use of empathy, sensitivity and confidentiality when communicating.
Person Specification
* NVQ level 3 in health and social care.
* Experience of delivering healthcare in the community.
* UK Driving License and access to a vehicle.
