Overview

We are excited to announce a band 6 Clinical Nurse Specialist position with the Renal Community Team. This is a unique opportunity for a passionate and experienced renal nurse to join a close-knit team to provide care in the community for a caseload of pre-dialysis and peritoneal dialysis patients. The team consists of 7 nurse specialists covering North, East, South and West Devon (including Exeter) and South, West and Mid-Somerset. The successful applicant will have a geographical area of which they are responsible for, with support from the other Renal Community Nurses and wider Renal MDT to manage their patients. This role requires flexibility and the ability to work well under pressure. The role includes managing a caseload of patients, education to patients and their families, and educating staff in the community and on the renal ward. Responsibilities also include Hepatitis B vaccination clinics, consultant peritoneal dialysis clinics, advanced kidney care clinics, emotional support to patients, liaising with primary care and running nurse-led local clinics.

We are looking for an experienced renal nurse to join the Renal Community team. Previous experience of managing a caseload of patients is desirable, though not essential. However an in-depth knowledge of all forms of renal replacement therapies is required. Experience looking after patients on home therapies, especially peritoneal dialysis, would also be expected.

The successful candidate should be an excellent communicator and approachable. They should be organised and able to work both independently and as part of a team. The successful candidate will be expected to:

* Assess, develop and implement specialist renal nursing care programmes and provide advice, education, support and care to people with chronic/end-stage kidney disease in the community setting.
* Provide specialist advice to primary and secondary care teams in geographical caseload area.
* Provide ongoing emotional and psychological support to patients and their families.
* Participate in and evaluate research and clinical audit in relation to specialty to inform and improve future practice.
* Provide education, information and advice to patients and their families regarding kidney disease and renal replacement therapy options.

Working Pattern: Mon-Fri 8-4pm, on-call rota weekends and bank holidays
