We are seeking someone with experience in the managementof minor ailments, chronic disease, and prescribing.
The sessions would be varied includingtelephone consultations and home visits as well as acute and plannedconsultations.
Main duties of the job
The ANP shall provide care forthe presenting patient from initial history taking, clinical assessment,diagnosis, treatment and evaluation of their care.
The role is unique in combiningadvanced nursing skill/practice with clinical diagnostic and treatment skills.
The ANP will workcollaboratively with the multidisciplinary general practice team to meet theneeds of patients, supporting the delivery of policy and procedures.

Job responsibilities
Assume a clinicalworkload dependant on the general practice population in collaboration with GPcolleagues.
Provides aholistic approach to the healthcare, drawing from both nursing and medicalmodels or health and illness where appropriate in order to screen for the earlysigns of disease and diagnose acute problems.
Demonstrateknowledge of the pathophysiology of acute and chronic conditions commonly seenin practice.
Accept referralfrom other disciplines unsupervised.
Undertake theindependent and autonomous assessment of undifferentiated and undiagnosedproblems with attention to physical, emotional, social, cultural and mentalhealth issues.
Uses soundjudgement in assessing conflicting priorities and needs.
Formulate adiagnosis utilising medical diagnostic problem-solving skills including historytaking, physical examination, invasive and non-invasiveprocedures/investigations.
Initiatesappropriate diagnostic strategies and therapeutic interventions includingradiology, scanning, and laboratory request, with attention to safety, costinvasiveness, acceptability and efficacy.
Develops with thepatient and family an ongoing, evidence based plan for health, with an emphasison preventative measures.
Interprets andevaluates diagnostic test results in liaison with the patient and their GP andinitiates treatment accordingly.
Evaluates thepatients condition, responds to therapeutic interventions, and modifies theplan of care accordingly for optimal outcomes.
Prescribe as anindependent nurse prescriber accordingly within the legal framework of nurseprescribing and the CCGs nurse prescribing policies.
Initiatesindependent and collaborative patient referral to statutory and non-statutoryorganisations including GPs; secondary care consultants, other members of theCCG, allied health care professional and voluntary agencies.
Makes use ofcomputer technology using automated information systems in order to supportdecision making, improve the quality and co-ordination of care and services andto enable faster and more certain communication within the CCG.
Evaluate aspectsof work using audit cycle to report services and standards of care to influencedecision making and planning.
Engages inclinical supervision and self-evaluation and uses this to improve care andpractice.
Manages change inresponses to altered dynamics within work and environment as influenced bypolitical, social and economic factors.
Maintain accuraterecords within the Clinical Governance Framework and the NMC Guidelines.
