Main Duties
As a registered nurse with experience in palliative care or a related field, you will work with our multi‑disciplinary team to provide exceptional, evidence‑based, person‑centred care for patients with complex, life‑limiting illnesses. You will manage a patient caseload, offer expert advice and interventions, and collaborate effectively with local GPs and Community Nurses as part of Hospiscare’s renowned specialist care.

Job Responsibilities
* Provide advanced specialist palliative care by contributing highly experienced clinical judgement and knowledge in complex palliative and end‑of‑life care to promote best practice for adults with life‑limiting illnesses.
* Demonstrate advanced communication skills and professionalism in all interactions with patients, carers and other professionals.
* Contribute to the daily clinical leadership of junior staff within the community team to ensure team effectiveness, cover and cohesion.
* Deliver education and learning, both internally and externally, to ensure evidence‑based practice.
* Attend and contribute to multidisciplinary, complex care and GSF or EOL meetings to promote best practice.
* Contribute to clinical governance and audit processes in order to support continuous quality improvement and learning.
* Assess and identify complex bereavement and communicate with the Supportive Care team to enable seamless handover into Bereavement Care Services.
* Maintain a safe environment for patients, visitors and employees, adhering to Hospiscare’s Infection Prevention and Control Policy.
* Obtain DBS Enhanced level check with Adults and Children Barred Lists.
Person Specification
* Essential Knowledge
* Detailed understanding of working with individuals at times of distress and conflict; understanding of key issues relating to End of Life Care, including policy developments; detailed knowledge of theoretical nursing models and their impact on care delivery.
* Essential Skills
* Expert practitioner skills; ability to manage a caseload effectively; highly developed communication and interpersonal skills; ability to deliver teaching programmes; capability to manage challenging circumstances; IT skills.

* Essential Qualifications
* Registered Nurse with current Nursing and Midwifery Council registration; Diploma in Palliative Care or related diploma‑level qualification; evidence of ongoing professional development.
* Additional Qualifications
* Advanced Communication Skills training; Teaching and mentorship/supervision qualification.
* Essential Experience
* Substantial experience of working within a palliative care/end‑of‑life environment; significant experience of effective clinical governance, audit and standard setting.
