In this role, you will assess needs, manage cases, and work alongside other professionals to deliver comprehensive, person-centred support.
What you will do:
1. Conduct comprehensive assessments for adults with learning disabilities, identifying risks and safeguarding needs.
2. Develop and implement tailored care plans that promote independence and respect service users’ dignity.
3. Manage and prioritise a complex caseload efficiently, maintaining accurate records and documentation.
4. Collaborate with health professionals, statutory bodies, and families to ensure coordinated service delivery.
5. Participate in multidisciplinary meetings, discharge planning, and safeguarding processes.
6. Identify and respond to safeguarding concerns according to local and national policies.
7. Prepare for and engage in supervision and team meetings, contributing to the development of colleagues.
8. Share knowledge to contribute to service evaluation and promote best practice in social work.
9. Be flexible, working across different locations and participating in duty rotas.
