Responsibilities
* Conduct thorough assessments of individuals’ needs for care and support, planning and implementing solutions that promote independence and positive change.
* Develop, review, and reassess care plans to ensure the evolving needs of each individual are met effectively.
* Oversee and engage in adult safeguarding enquiries and collaborate with other professionals to develop safeguarding plans tailored to the wishes and circumstances of individuals.
* Utilize a strengths-based approach to foster self-determination and resilience in those you support.
* Make professional judgments and decisions within the legal and policy frameworks.
