The role of the PCN Project Support Officer will support various services and contractual requirements run within the PCN, including specific project work, care coordination and standing administrative responsibilities. They will also assist the PCN Management Team in maintaining robust data collection and reporting processes and ensure compliance with relevant procedures and policies.
Please note that this job requires travel between all 5 PCN practices and working at multiple sites.

Main duties of the job
ProjectSupport
Provideproject support for existing and future projects of LCS PCN
Supportthe Enhanced Access Service
Developand run searches on SystmOne clinical system
Undertakedata analysis and assist with a range of services evaluation monitoring
Engagewith relevant clinicians for the services and stakeholder practices of the PCN
Supportthe proactive identification and management of patient caseloads in line withthe PCN DES
Workingcollaboratively with colleagues across the PCN, Health, Social Care and VCSEsectors providing patient centred, integrated care.
Assistthe PCN Management Team with collation and production of project and servicedocumentation.
Developand maintain databases and systems for monitoring and evaluation
Engagewith the PCN practices to present findings, recommendations and to deliveractions agreed
Supporttraining of staff as appropriate to improve service outcomes
AdministrationTasks
Supportthe administrative tasks of services and PCN DES contract delivery thatincludes appointment and referral management for patients
Provideadministrative support to the PCN Board, including agenda setting, roombooking, informing people of their follow-up action points
Supportthe PCN Management Team in the day to day running of the PCN, ensuringcompliance with relevant policies and procedures
Responsiblefor developing and maintaining systems for storage and retrieval ofinformation, ensuring information is accessible to other members of the PCN

Location
This role will require regular travel amongst the PCN member practicesand business meeting locations within the LCS locality.

Qualifications
* Evidence of a sound education to A level standard or equivalent
Skills and Competencies
* Computer literate with an ability to use SystmOne and Microsoft office packages.
* Has attention to detail, able to work accurately
* Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines
* Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience.
* An excellent understanding of data protection and confidentiality issues
* Understand the aims of current healthcare policy within the PCN.
* Able to analyse, interpret and present data.
* Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems.
Experience
