Overview
As a Clinical Pharmacist specializing in Oncology & Haematology, the primary role is to deliver high-quality and responsive clinical pharmacy services to patients on allocated wards. This includes assisting junior pharmacists and Medicines Management technicians, facilitating efficient discharge planning, and ensuring safe transfer and continuity of care.
Key Responsibilities
* Discharge Planning: Review and transcribe patients’ discharge treatment. Facilitate timely dispensing of discharge medication. Liaise with healthcare practitioners for safe transfer of care.
* Prescribing: Undertake prescribing as a Pharmacist independent Prescriber. Lead efficient organization of the dispensary, ensuring a safe work environment.
* Training and Support: Train and support staff in formulary control and adherence to policies. Provide tailored counseling to patients on complex therapy regimens.
* Service Delivery: Monitor service delivery and cover Outpatients dispensary when required. Contribute to policies, procedures, and service planning.
* Clinical Checks and Anomalies: Undertake clinical checks of treatment regimens. Resolve anomalies with prescribers and contribute to dispensing.
* Emergency Duty Commitment: Address issues arising during weekend service hours. Contribute to out-of-hours emergency Pharmacy service.
* Specialist Pharmacy Service: Provide highly specialist clinical pharmacy advice in Haematology and Oncology. Liaise with patients, attend multi-disciplinary team meetings, and contribute to the development of the chemotherapy management system.
* Aseptics and Clinical Trials: Check treatment regimens and final products in Aseptics Unit. Assist with clinical trials implementation and running.
* Training and Support: Train junior pharmacists, Foundation Student Pharmacists, and technicians. Act as a link to the Nutrition Team, supporting Clinical Nutrition.
* Continuing Professional Development: Demonstrate commitment to ongoing professional development.
