Castleton Health Centre is looking for an enthusiastic and proactive Practice Nurse to join our friendly team. We value teamwork, mutual support and a friendly working environment. We are looking for NMC registered professional, with General Practice Nurse experience.
Main duties of the job
The post holder will be an experienced Practice Nurse, who acting within their professional boundaries, will provide high quality care for patients. The role includes the typical practice nurse duties including: diabetic & respiratory care (asthma & COPD), smears, immunisation, contraception, health reviews, etc.
The individual will provide nursing service/treatments to patients independently or in participation with general practitioners working within the boundaries of the NMC Code of Conduct to agreed practice standards and protocols.

Job responsibilities
Key Responsibilities
* Deliver a wide range of nursing services, including immunisations, and cervical screening
* Lead and support the management of chronic diseases such as asthma, COPD, diabetes, and hypertension through structured clinics and patient reviews
* Undertake chronic disease monitoring, including observations, blood tests, and interpretation of results within scope of practice
* Develop, implement, and review individualised care plans for patients with long-term conditions
* Provide education and self-management support to patients, empowering them to take an active role in managing their health
* Conduct NHS health checks, screening programmes, and preventative care initiatives
* Promote healthy lifestyles, including advice on smoking cessation, diet, exercise, and weight management
* Administer vaccinations in line with national immunisation schedules
* Maintain accurate and up-to-date clinical records in accordance with practice policies
* Work collaboratively with GPs and the wider multidisciplinary team to ensure coordinated care
* Professional Responsibilities
o Maintain valid registration with the Nursing and Midwifery Council (NMC)
o Work in accordance with the NMC Code and professional guidelines
o Ensure patient confidentiality and compliance with data protection regulations
o Follow safeguarding policies and procedures
o Participate in clinical audits, quality improvement, and ongoing training/CPD

o Strong clinical knowledge, particularly in long-term condition management
o Excellent communication and patient education skills
o Ability to manage clinics independently and prioritise workload effectively

o Good IT and record-keeping skills

This job description will be reviewed during annual appraisals and may be altered following consultation with the post holder.
Experience
* Must have experience working in Primary Care as a Practice Nurse
* At least 2 years experience working in Primary care as a Practice Nurse
Qualifications
* Nursing Diploma/ Degree and NMC registration Experience in General Practice environment. Knowledge and application of supporting patients with long-term conditions Cervical screening, family planning and immunisations experience Insight and understanding of current issues in nursing and NHS Understanding of computer based medical records management Disclosure and Barring Service Check Full clean driving license
* Independent non- medical prescriber Mentor/teaching qualification Recognised Infection Control Qualification
