Bradford City 5 PCN are seeking 2 compassionate and proactive Children and Young Persons Social Prescribing Link Workers (SPLW) to join our well established team consisting of Social Prescribing Link Workers (Adults), Health and Wellbeing Coaches and Mental Health Practitioner. Thisrole is central to supporting children, young people and their families/Carers empowering them by connecting them with non-medicalcommunity support.
You will help individuals improve their health and wellbeingby addressing wider social determinants and enabling them to focus on whatmatters to them.
This is a person-centred role, working with children and young people fromdiverse backgrounds, including those who may be socially isolated, living withlong-term conditions, or facing complex social challenges.
This is a full-time, practice-based role, but applicationsfrom those wishing to work part time will be considered.
While this role is fixed term (12 months) any potential for extension will be subject to availability of further funding
Main duties of the job
The role is primarily to work with children and young peopleaged up to 19 years (up to 24 years old for users with Learning Difficulties)but may also involve work with their parents/carers. You will work closely withthe GP practices, schools and colleges (if required) and a variety ofestablished services to provide a range of support for children, young peopleand their families and deliver the appropriate level of support at the righttime.

Accept referrals from a wide range of agencies including GPpractices within primary care networks.
Signpost young people and their families to appropriatecommunity-based services.

Co-produce a personalised support plan to improve health andwellbeing, introducing, connecting, and supporting young people to communitygroups and statutory services.
Help young people identify the wider issues that impact ontheir health and wellbeing, such as debt, poor housing, being unemployed,loneliness and caring responsibilities.
Form links with local Schools and other communityenterprises.
Provide earlier intervention whilst waiting or avoiding theneed to wait for triage from other services.
Work closely and communicate with other agency partners suchas schools, Community Development Workers, Local Area Coordinators, TargetedYouth Service and Family Support Teams when necessary to ensure the localsystem is working in a joined-up way to provide the young person(s) withholistic care.
Where necessary, physically introduce people to communitygroups, activities, and statutory services, ensuring they are comfortable.Follow up to ensure they are happy, able to engage, included and receiving goodsupport.
Work sensitively with young people to capture keyinformation, enabling tracking of the impact of social prescribing on theirhealth and wellbeing.
Manage and prioritise your own caseload, in accordance withthe needs, priorities and support required by individuals on the caseload.
Work closely with safeguarding leads to ensure any issuesand concerns are escalated and dealt with quickly and effectively.
Refer young people back to other healthprofessionals/agencies, when their needs are beyond the scope of the linkworker role e.g., when there is a mental health need requiring a qualifiedpractitioner.
Attend regular MDT meetings (multi-disciplinaryteam).
Build relationships with key staff in GP practices withinthe local Primary Care Network (PCN), attending relevant meetings, becomingpart of the wider network team, giving information and feedback on socialprescribing.
Be able to work flexibly in-hours and out of hours.

Record interactions and outcomes using relevant NHSclinical/IT systems (e.g. EMIS, Joy)
Provide regular feedback to referring teams and supportservice monitoring and evaluation.
Develop youth-friendly approaches tailored to thedevelopmental, emotional, and social needs of children and young people.
Work collaboratively with families, carers, schools, youthworkers, and childrens services.
Champion the voice of young people, ensuring support isshaped by their priorities and goals.
Promote the service within schools, youth organisations, andthrough community events.

* Experience of working with children and young people, childrens health and social care, learning support or public health/health improvement (including unpaid work)
