We are looking for an enthusiastic, courteous and friendly practice nurse to provide a range of nursing services to our 11000 patients,and join our friendly, supportive general practice team. The individual willjoin our fantastic team - flexible hours up to full time 37.5 hours and part-time workingwill be considered.
We are keen on staff training and support and have daily staff coffee breaks and 4 in house training sessions per year where the practice is closed.
Main duties of the job
* To undertake and oversee chronic disease management (i.e. asthma, diabetes, hypertension, coronary heart disease, COPD), incorporating progress-monitoring, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of nursing practice. To ensure all clinical domains and indicators for disease monitoring and health promotion (QAIF) are met.
* To facilitate and participate in multidisciplinary education within the practice. To support students of any discipline undertaking a work placement at the practice including our Academy nurse trainees, medical students and GP trainees.

Job responsibilities
* To undertake and oversee chronic disease management (i.e. asthma, diabetes, hypertension, coronary heart disease, COPD), incorporating progress-monitoring, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of nursing practice. To ensure all clinical domains and indicators for disease monitoring and health promotion (QAIF) are met,
* To facilitate and participate in multidisciplinary education within the practice. To support students of any discipline undertaking a work placement at the practice including our Academy nurse trainees, medical students and GP trainees.
* Able to perform basic health checks and test including BP monitoring- including 24 hours, urinalysis, spirometry, phlebotomy, perform other routine laboratory tests, ECGs but these would not be the main scope of work.
* Womens health care including cervical smears, vaginal swabs, contraception counselling, administration and monitoring and menopause management.
* All aspects of wound care including doppler assessments and compression dressings, packing and management of infections.
* Administer injections including flu, covid, travel injections and baby vaccinations as per patient group direction
Administration
* Participate in the administrative and professional responsibilities of the practice team including audits and review of protocols and procedures
* Attend and participate in monthly clinical meetings
* Restocking and maintenance of clinical areas, regular checks on emergency equipment, drug and vaccine stock take and re-ordering on monthly basis.

Experience
* The candidate must (this list is not exhaustive):
* be able to communicate clearly and professionally
* have good team working skills and a flexible approach
* ideally have proven experience as a Practice Nurse
* be proficient with computers and record keeping (EMIS or similar experience is required)
* Experience in Primary Care and able to undertake and oversee chronic disease management (i.e. asthma, diabetes, hypertension, coronary heart disease, COPD), incorporating progress-monitoring, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of nursing practice. To ensure all clinical domains and indicators for disease monitoring and health promotion (QAIF) are met. Proficient in adult and child vaccination programmes.
