What is the job role?
This is an exciting opportunity to work with individuals experiencing homelessness, mental health crises, and at risk of psychiatric inpatient admission. You will join a busy Home Treatment Team promoting the bio-psycho-social model of mental disorder, supporting the social model of practice. Your work will be stimulating and challenging, working closely with NHS and Local Authority Homelessness Services, including accommodation providers, and an experienced team of Social Workers, AMHPs, Psychiatrists, Nurses, Support Workers, and other partner agencies.
You will be expected to:
1. Support individuals requiring urgent social support, focusing on preventing homelessness and/or sustaining accommodation.
2. Complete personalized assessments of social needs.
3. Conduct assessments under the Care Act (2014) and arrange social interventions aimed at recovery and wellness.
4. Collaborate with statutory and third sector colleagues to assess, identify, and implement interventions based on social and recovery models.
5. Always offer compassionate and respectful responses that inspire hope and recovery.
This post is exempted under the Rehabilitation of Offenders Act 1974, and appointment is conditional upon a satisfactory DBS check.

* Knowledge of social issues impacting individuals with mental disorders and available interventions.
* Understanding of homelessness and associated challenges.
* Ability to assess risk and escalate appropriately.
* Creativity in exploring social solutions, including direct payments.
* Ability to mobilize relevant agencies and form purposeful relationships with individuals, families, carers, and partner agencies.
