Role Overview
In response to national concerns about the care of people with learning disabilities and autism, the Trust has developed an out‑of‑area and complex case manager role. This role ensures that individuals placed out of area are regularly reviewed to maintain high standards of care and minimises unnecessary hospital admission.
You will work with specialist social workers, mental health colleagues and other system partners to keep care within the home setting whenever possible, supporting some of our most vulnerable and misunderstood clients.
Key Responsibilities
* Work autonomously to support and coordinate the case management of individuals with highly complex health and social care needs, ensuring service users remain at the heart of the process.
* Allocate and monitor day‑to‑day case management expertise for Mental Health Act eligible patients with highly complex needs, including monitoring high‑cost care packages and 1:1 provision.
* Define current processes, facilitate discussions and advise colleagues on safeguarding, Court of Protection, mental capacity and best‑interest decisions.
* Make decisions concerning complex mental health case‑management issues in line with national guidance and with the support of senior colleagues when needed.
* Ensure delivery of a full, high‑quality NHS health‑care service and maintain best outcomes for individuals.
* Travel independently between sites within the Trust, responding to changing case‑management needs.
* Collaborate with system partners, including social workers and mental‑health colleagues, to avoid hospital admissions and secure timely discharges when admission is unavoidable.
