We are looking for a passionate, qualified Level 1 Social Worker to join our Adult Social Care team within the Newcastle District. We are a community-based team within the older persons/physical disability sector, working with adults over 18 years of age with a range of physical and mental health needs. We are pleased to offer a specialist Social Work Learning Academy (SWLA), an internal training program that offers ongoing social work training and professional development. On appointment, you will be enrolled in the SWLA and aligned with the appropriate British Association of Social Work Professional Capabilities Framework. The SWLA 'stage' (Stage 1: ASYE, Stage 2: Social Worker, Stage 3: Advanced Social Worker) will be determined based on your prior CPD and relevant experience. The post holder will be responsible for their own professional development.

As a registered social worker, you will play a key role in supporting the delivery of personalisation and social inclusion agendas, ensuring quality public-facing services, including health and social care. Your responsibilities will include:
1. Conducting a range of assessments, including holistic needs-led assessments, care and support planning, reviews, and initiating multi-disciplinary assessments where appropriate.
2. Being an integrated and supportive team member, planning, actioning, monitoring, and evaluating work under supervision from an experienced Social Worker and the Advanced Social Work Practitioner lead.
3. Collaborating with other Trust services to provide seamless care for adults and carers, focusing on safe and effective transitions.
4. Participating in duty/ACCESS or priority rota to carry out assessments for health and social care services for adults and carers, utilizing various assessment tools, including a Risk Screening Tool.
