Our reablement hub is a 30-bed service supporting adults as they rebuild skills, confidence and independence following illness or crisis. We work closely with families and a wide range of health and social care professionals to ensure every person receives flexible, person-centred support. What You’ll Be Doing As a Reablement Assistant, you’ll be at the heart of our service, supporting adults to live as independently as possible. Your day-to-day role will include: Supporting people with their physical, social and emotional needs Helping individuals regain and maintain daily living skills, from personal care to kitchen and household tasks Working as part of an integrated team alongside therapists, nurses, social workers and other professionals Encouraging service users to maximise their potential and confidence Contributing to assessments and person-centred support plans Promoting dignity, choice, independence and respect at all times Supporting individuals to take part in activities, therapies and meaningful routines Using manual handling and other equipment safely Maintaining accurate records and communicating changes promptly Providing excellent customer service and upholding organisational values⭐ What We’re Looking For Essential: Experience in an adult social care setting Experience supporting people to build independence Good written and verbal communication skills Ability to follow reablement programmes Strong listening skills and empathy Willingness to work flexibly across a 3-week rolling rota NVQ Level 3 in Health & Social Care or willingness to work towards it Commitment to training, supervision and development Ability to work in line with safeguarding, equality and health & safety standards Enhanced DBS clearanceDesirable: Manual Handling certificate Experience delivering person-centred care Strong teamwork skills Commitment to organisational vision and values Why Join Us?
