Now Recruiting: Social Prescribing Link Workers (SPLWs)
Be the bridge between clinical care and community wellbeing.
Social prescribing is a powerful tool that helps people take control of their health by connecting them to non-medical support in their communities. As a Social Prescribing Link Worker, you’ll take a holistic, person-centred approach—helping individuals navigate life’s challenges and access the resources they need to thrive.
What You’ll Do:
* Manage a caseload of individuals with complex social needs
* Conduct comprehensive assessments and develop personalised care plans
* Identify barriers to wellbeing and connect people to community-based support
* Use coaching techniques to empower individuals to take charge of their health
* Build relationships with local services and help establish new community groups
What You Bring:
Skills:
* Coaching techniques (goal setting, behaviour change)
* Active listening and rapport-building
* Independent decision-making and IT proficiency
* Team collaboration within primary care settings
Experience:
* Multidisciplinary team environments
* Health, social care, or community development background
* Cross-sector partnership working
* Understanding of social determinants of health
Qualities:

Qualifications:
* Essential: NVQ Level 3 in Social Prescribing (or working towards)
* Desirable: Training in health coaching, motivational interviewing, and behaviour change
This role covers Doncaster area DN1 and DN12, A mixture of Practice work and remote work.
