The Llyn district nursing team are looking to recruit a dynamic Band 4 Assistant Practitioner to join our team. The Assistant Practitioner role will also require the post holder to be an effective communicator and practice in accordance with the standards and statutory requirements and operate within the boundaries of the role and essential competencies.

Main Duties
The Assistant Practitioner will work as part of a designated team to deliver evidence‑based, high‑quality clinical care, focusing on the direct needs of patients, including those with chronic conditions.
* Ensure effective communication with patients, carers, multidisciplinary team and the wider health and social care community.
* Practice in accordance with Health Board standards and statutory requirements, operating within the boundaries of the role and assessed competencies.
* Use personal effectiveness and knowledge to contribute to the development and delivery of a quality service.
* Work proactively as a member of the multi‑disciplinary and multi‑professional healthcare team, supporting registered practitioners.
* Manage a small defined caseload of patients under the supervision of the registered nurse.
Qualifications
* Hold the QCF Level 4 Qualification – Foundation Degree in Health Care Level 4 or are working towards it.
* Desirable: Ability to speak Welsh; Welsh and/or English speakers are equally welcome to apply.
