Job summary
We are seeking an experienced and enthusiastic Band 6 Community Psychiatric Nurse to work within community services in the borough of Merton for South West London and St George's Mental Health NHS Trust, within the Community Recovery Support Team. The post offers a chance to work as an integrated member of a dedicated community team, working with adults experiencing mental health issues. Our community services are innovative and have recently been reconfigured to provide specific recovery focused interventions to people with mental health issues accessing secondary care. The focus is on providing time limited interventions to enable individuals to maintain their lives with limited intervention; to understand and develop strategies to manage symptoms they may be experiencing, while working in partnership to establish their own personal recovery action plans.

Job responsibilities
* Be responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and carers.
* Maintain a high standard of individualised client care, recognising factors that affect mental, emotional and physical well-being and provide advice and support.
* Act as a care coordinator in assessing clients’ needs and strengths, ensuring care plans are implemented and regularly reviewed/updated.
* Develop and maintain a high standard of practice using psychosocial approaches within a health and psycho-social framework.
* Communicate and liaise effectively with relevant agencies to provide an integrated programme of care.
* Implement and/or participate in crisis intervention measures with the client/family or with other agencies.
* Supervise and monitor the effects and side-effects of prescribed medication, providing clear advice in line with Trust Policy and NMC standards.
* Organise and participate in admission, discharge and transfer of clients in accordance with Trust Policies.
* Provide clients and carers with up-to-date information on services, how to express views, the complaints procedure, and what to expect from the service.
* Attend weekly clinical meetings to discuss caseloads and referrals and provide feedback on assessments.
* Develop and maintain effective working relationships with in-patient facilities regarding admission and discharge of clients.
* Facilitate carers assessments and provide appropriate care plans to meet identified needs and monitor efficacy.
* Have knowledge and application of key legislation and policy such as the Mental Health Act, Care Programme Approach, Capacity Act and the Care Act.
* Participate in the running of clozapine and depot clinics.

Essential
* Registered mental health nurse
* Mentorship for Contemporary Practice
* Knowledge of NICE guidelines and evidence-based interventions
* Knowledge of counselling, group, cognitive and behavioural skills
* Experience in clinical assessment, risk assessment, mental state examination, capacity assessment and crisis management
* Good verbal and written communication and report-writing skills
* Knowledge of current government policy and relevant legislation regarding community mental health care
