Social Worker - Learning Disability & Autism Service (Adults) at Somerset Council. Jobs located in Mendip, Bridgwater or Taunton. Full time, 37 hours per week. Starting salary £37,280 rising to £41,771 per annum. Hybrid working opportunities. 30 days annual leave entitlement.
Responsibilities
* Work as part of a small, supportive Learning Disability and Autism service in the West or East of Somerset, completing Care Act assessments, reviews, mental capacity assessments, safeguarding enquiries and Court of Protection work.
* Participate in a duty system across the county and engage in different ways of working with people we support, including motivational interviewing, crisis intervention and positive risk management.
* Collaborate with colleagues and specialist professionals to contribute to decision making through weekly Peer Forum discussions, supporting requests for increases to personal budgets and packages of care.
* Work with other specialist teams within the Authority (Preparing for Adulthood, Commissioning, Occupational Therapy, DOLS, Mental Health, Safeguarding, CHC) and build links with LD Health colleagues in Somerset Foundation Trust.
* Advocate a strength-based, personalised approach using the three conversations model to help people live more independent and fulfilling lives.
* Support the development of the Adult Social Care service and work with individuals to achieve their goals and desired outcomes.
What kind of experience or qualifications do I need?
* Be a qualified Social Worker and be registered with Social Work England.
* Have access to/use of a vehicle is an essential requirement for this role.
