We are looking for an experienced practice nurse to join the Southend Coastal Surgeries clinical team. The post is part-time offering 2 full day shifts a week.
The key purpose of the role is to assess, plan and evaluate nursing care needs for the practice patients.
To participate in the planning, development and management of the surgery alongside the GP and other team members.
Main duties of the job
Towork closely with our general practitioners (GPs) to deliver care in a primarycare setting. Responsibilities includescreening and referring patients,carrying out immunisation and vaccination programmes, managing chronicdiseases, and promoting healthy lifestyles.Serving as thefirst point of contactfor patients enteringthe healthcare system and provide holistic care across all age groups. In addition topreventive care, womenshealth, respiratory and cardiovascular management, and care for patients withlearning disabilities.

Working togetherin partnership to deliver services to their patient population across bothpractice sites, the practice is clinically lead by a GP and an AdvancedClinical Practitioner, the partners are supported by a diverse clinical teammade up of salaried GPs and part time locum GPs, Advanced Nurse Practitioners,Associate Nurse Practitioners, Prescribing Paramedics, Practice nurses, GPNurse Assistant, Health Care Assistants, a Lead Pharmacy Technician, PharmacyTechnicians, and a Prescription Clerks.
Theadministration team comprise of Care Navigators, Care Co-Ordinators, Medicalsecretaries, administration staff, Assistant Manager, Practice Manager, Primarycare Development Manager and Business Manager with a list size combined of c15,500 patients.

Job responsibilities
* Responding to, dealing with and treating patients with respect,patience, diplomacy and tact.
* Receiving patients who present with undiagnosed clinical needs, makingclinical assessments of those needs and then developing, implementing andevaluating programmes of care to meet those needs.
* Making professionally autonomous decisions relating to patientspresenting with problems for which you will have sole responsibility and beaccountable for.
* Managing challenging consultations effectively with the patients andothers in sensitive or difficult situations such as disability, distressed andangry people and third party consultations.
* Managing and prioritising own caseload effectively, working on your owninitiative within agreed clinical boundaries and being confident in owndecision-making processes.
* Performing duties and supporting research following appropriatetraining, practice guidelines and evidence-based practice.
* Admitting patients to the primary and secondary health care systems, byinitiating and carrying out treatment or by referral to other clinicalprofessionals.
* Screening patients for disease risk factors and early symptoms ofdisease.
* Participating in chronic disease management clinics within the practiceand providing practical assistance and professional advice to achieve minimumstandards of care.
* Facilitating patient involvement in care planning by providing accurateclinical information which empowers them to be involved in making informeddecisions about their health, lifestyle and choices.
* Providing educational opportunities relating to health care and healthpromotion.
* Providing advice and specialist information to the health community,nursing and other professional colleagues relating to the particular healthcare needs of the key client groups in the Surgery.
* Evaluating patients and treatments and discharging patients from ongoingcare where appropriate.
* Participating with the Surgery team, interacting and combining withother health care professionals both internally and externally to formulatepolicies, provide a comprehensive service to patients, develop the Surgery andbuild networks for the effective management and care of patients.
* Support the Surgery in achieving quality indicators and targets.
* Working closely with Care and Nursing Homes concerning treatment andongoing case management of registered patients with the homes.
* Visiting patients at home as necessary.
* Maintaining the standards, quality assessments and audit of practice inorder to improve the standards in line with the clinical governance policy ofthe practice.
* Taking part in appropriate clinical supervision to maintain and improvestandards of care, and to be regularly monitored for quality and consistency ofservice.
* Supervising, deploying and developing staff in the Surgery whereappropriate to encourage a skill mix.
* Being involved in the recruitment, selection and appointment of Surgerystaff.
* Being responsible for efficient and effective use of resources andproviding consistently high-quality standards of clinical care.
* Attending meetings at the Surgery as necessary.
* Any other reasonable duties required to support the needs of theSurgery.
Knowledge and Skills
* Willing to develop clinical skills and competency relevant to the role
* Excellent communication skills, both written and verbal
* Strong organisational skills
* Excellent keyboard and computer skills
* Competent in childhood immunisation, womens health, wound care, and general nursing
* Knowledge of health promotion strategies
Qualifications
* 1) Registered General Nurse
* 2) Current NMC Registration
* 3) Demonstrable commitment to professional development
* 4) Eligible to work in the UK
Experience
* 1) Experience of working in accordance with the NMC Code of Professional conduct
* 2) Minimum of 2 years post registration experience
* 3) Previous experience of working in General Practice and/or A&E
* 1) 5 years post registration experience
