An exciting opportunity has arisen for the Adult Community Nursing Service. We are currently seeking Band 5 Community Nurses to work across the Hounslow areas, based at the Bedfont Clinic, Imperial Road. The service operates from 07:00 to 19:00, seven days a week, working alternate weekends, with shifts of 07:00-15:00 (early), 08:30-16:30 (day), and 11:00-19:00 (late).
Collaborative working with our integrated rapid response teams, community matrons, and social service colleagues is essential to ensure seamless care for our patient group.
The role involves providing high-quality, skilled nursing care and assisting the Community Nursing Team Manager in planning, implementing, and evaluating holistic care needs of patients, families, and carers.
You will deliver high-quality nursing care to housebound patients with complex needs, enabling them to remain in their own homes. Working closely with specialist nurses, allied health, and social care professionals, the aim is to meet patient needs without unnecessary hospital attendance or admission.
This will involve working independently within a geographical area, including assessments, reviews, planning, implementation, monitoring, and evaluation of individual care plans. Flexibility to support a corporate approach to patient care as needed is expected.
