This role would suit an Allied Health Care Professional, especially a Paramedic, Physiotherapist, or a Specialist Nurse. The successful candidate will play an integral part in assessing, supporting, and treating people with a range of respiratory conditions caring for and improving the standard of care for respiratory patients.
The successful candidate will have the opportunity to work within a variety of settings. The award-winning team is based in the Chest Clinic at King Edward VII Hospital in Windsor, but we see patients as hospital in-patients, in clinic and in their own homes. The focus is supporting people to manage their condition within the most appropriate setting. No two days are the same.
Having launched in October 2016 we are now expanding the team so the AIR service will provide respiratory care across East Berkshire in and out of the hospital setting, delivering acute respiratory care for patients with Asthma, COPD, Home Oxygen, Interstitial Lung Disease, Bronchiectasis, Respiratory Physiotherapy and Pulmonary Rehabilitation.
The role is a predominantly 0.8 WTE Monday - Friday with the hours being 9-5, to be agreed. There is some requirement to work 8-4 at times and the potential for an occasional Saturday rotation.
Main duties of the job Assessing and reviewing respiratory patients at home as part of the Supportive discharge, admission prevention or acute respiratory virtual ward.
Visit patients in their homes to assess their progress, using advanced assessment skills; plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care
Using effective counselling/educational skills which may involve breaking bad news on diagnosis or prognosis of a chronic condition.
To contribute to palliative care of respiratory patients as they approach end of life.
Assessing and reviewing patients who are on long term oxygen therapy within their usual place of residence
To provide oxygen assessment to those patients referred to the service within the clinic setting, considering the most appropriate modality should oxygen be required.
Assess acutely unwell patients with respiratory illness in particular COPD who have been admitted to the Acute Hospitals (WPH) with the view to supporting discharge. This will involve reviews of their physical needs, their social and psychological circumstances and will require advanced assessment skills in those areas

Job responsibilities The must haves for this role:
Registered Nurse Part 1,2,12 or equivalent qualification in allied health care professions
Full driving licence and access to a car for use to travel across sites and perform domiciliary visits.
Minimum 2 years relevant clinical experience in the same or related specialty OR At least 2yrs recent clinical experience in the acute setting
Able to work autonomously and within a team
Evidence of continuing professional development

Person Specification Education/Qualifications Registered Nurse or Allied Health Care Professional
Relevant specialist post basic programme in respiratory / experience or relevant community nursing qualification
First degree in nursing, acute skills, patient assessment or demonstrable experience at this level
Teaching / assessment programme
Diploma in respiratory care (COPD / Asthma) or willing to complete
Previous Experience Minimum 2 years relevant clinical experience in the same or related specialty OR At least 2yrs recent clinical experience in the acute setting
Experience in managing the acutely unwell respiratory patient in the hospital setting
Evidence of ability to work collaboratively
Previous experience in audit and evaluation
Experience in acting as a clinical supervisor and mentor to others
CBG / ABG interpretation
Previous community nursing experience
Non Medical prescribing course
Knowledge, Skills & Abilities Demonstrates up to date knowledge and expertise in general nursing
Knowledge of key professional issues and NMC guidelines and codes, relating to professional practice
Awareness of community working and structure
Knowledge of the COPD and asthma outcomes strategy, NICE & BTS guidelines
Clinical knowledge & skills in respiratory care
Able to work autonomously and within a team
Evidence of continuing professional development
Previous experience of formal / informal teaching of patients & staff
Demonstrates skilled written and verbal communication skills
