Join our award-winning specialist service as a Band 6 Outreach Nurse at Luther Street Medical Centre. The role aims to deliver excellent care directly to communities in need, working in hostels, day centres, temporary accommodation and street settings to support people who cannot easily access healthcare.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Responsibilities
* Delivery of preventative primary care and pre‑hospital health interventions to the homeless and vulnerable groups – focussing on outreach work.
* Effective operation of a nursing outreach service in community settings such as hostels, voluntary groups, homeless day centres and temporary accommodation.
* Working collaboratively with staff in community settings and the Oxfordshire Health and Homelessness Inclusion Team to support patient health.
