About Us
Connect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part‑time basis. It delivers extended access primary care services and a Physio First service in partnership with the local acute and community trust, and supports the development of primary care networks and delivery of training to the general practice workforce.

Job Responsibilities
* Undertake medicines reconciliation in accordance with practice protocols and update patients’ medical records / prescribing accordingly.
* Support the achievement of the practice’s prescribing Quality and Outcomes Framework targets.
* Work with practices to review and develop repeat prescribing protocols / SOPs to improve efficiency and effectiveness of repeat prescribing systems.
* Provide advice to GPs, staff and patients in changes to prescribing to support safety, quality and cost‑effectiveness; attend MDT meetings if required.
* Participate in medication audits.
* Assist in monitoring and managing prescribing budgets and liaise with the medicines management team at the CCG.
* Prepare evidence‑based resources and information to support rational cost‑effective prescribing.
* Support practices to provide medication reviews, patient information leaflets, medicine awareness and undertake community visits as appropriate.
* Take a proactive approach to safe handling of prescriptions, assess risk and make recommendations for improvement.
* Respond to medication queries from patients and staff professionally, referring to the appropriate GP in accordance with practice protocols.
* Advise on the sourcing and safe management of medicines as appropriate.
* Be responsible for the organisation, planning and management of own workload to meet deadlines.
Additional Administrative Responsibilities
* Take messages and pass on information as appropriate.
* Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures.
* Initiate contact with and respond to requests from patients, other team members and associated healthcare professionals and providers.
* Help to maintain a positive, patient‑focused culture and a supportive culture across the whole of the practice team.
* Perform any other duties commensurate with this position under direction of Partners / Practice Managers, depending on current workload.
Person Specification
* Essential:
o Registered with the GPhC as a Pharmacy Technician.
o Excellent communication skills both verbal and written.
* Desirable:
o PCPEP.
o CPD evidence.
o Experience of using SystemOne.
o Experience working in GP practice, Primary Care, hospital or community pharmacy.
