Job Overview
We are seeking to appoint an experienced senior palliative clinical nurse specialist to work within the Community Specialist Palliative Care Team within Cheshire Wirral partnership. The team consists of Clinical Nurse specialists ,Occupational Therapy, Physiotherapists, a Therapy Assistant and administration staff.
Experience of working in the community is desirable as the role is based supporting patients in their own homes.
We are looking for an experienced and well-motivated Clinical nurse specialist with excellent communication, organisation and leadership skills. The successful candidate will receive a full supportive induction when they join the team and will work closely with the wider MDT.
The post is part time 30 hours per week.
Main duties of the job

The post holder will provide holistic care and support to patients with complex palliative care needs with malignant or non-malignant life limiting conditions, working autonomously and as part of a team. They will deliver care to patients registered with a West Cheshire GP in their own home. The post holder will provide advice, education and support to patients, their carers and other health care professionals. They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations whilst meeting the holistic care needs of the patient.

Person Specification Qualifications Essential Criteria
* Registered General Nurse
* First level degree in related subject
* Clinical examination level 7 module or Differential Diagnosis level 7 module or equivalent
* Evidence of Masters level learning or prepared to work towards
* Non-medical prescribing (V300) qualification, or willingness to complete within 18 months of appointment
* Post-graduate qualification in palliative care or relevant subject
* Evidence of regular continued professional development (CPD)
* Advanced communications skills training or prepared to work towards
* Recognised teaching qualification or prepared to work towards
Desirable criteria
* Research qualification

* Expert in palliative care nursing

* Teaching/assessment and presentation skills
* Exemplary written communication skills

* Relevant post registration experience, at Band 6 or above, in palliative care.

* Previous experience of working as a clinical nurse specialist
