Warwickshire County Council offers a hospital social care team approach to working with older people in a hospital acute setting. The Hospital Social Care team is pioneering a strengths-based approach and focuses on universal services. The teams operate across three hospital sites in Warwickshire County Council/Coventry. We are dedicated and supportive, aiming to respond quickly and provide strengths-based assessments and support to customers and their carers in a fast-paced environment.
Team and roles
The teams include Social Workers, Social Care Workers, Admin, Trusted Assessors, Team Managers & Senior Managers.
Role overview
Joining the established Hospital Team, you will provide a social work service to a high professional standard. You will manage a caseload and travel within North and South Warwickshire as required, working with older adults, their families and unpaid carers. You will undertake and record robust assessments and care and support planning. You will communicate with a wide variety of customers, communities, health partners and agencies, and work on your own initiative, often in pressurised situations.
Team members are supported by colleagues and Team Managers who provide regular supervision and day-to-day support. Staff have access to regular training provided by Warwickshire County Council.

Key requirements
* To be successful in this role you must have a professional Social Work qualification and be registered with the Health and Care Professions Council.
* Be able to work 5 days over 7 to support a streamlined service and timely discharges. The hospital teams currently work one weekend in a month (4 weekly rota). Candidates should be prepared to work weekends.
* To work closely with the Multidisciplinary Health & Social Care team.
* To participate fully in the duties of the team, including team meetings and other meetings determined by the Team Manager and/or Service Manager.
* Driver’s licence is essential for the social work role as you will be expected to complete reviews post-discharge, especially in care homes.
* You will have a dynamic approach and embrace our new Strength-based way of working.
* You will be used to making difficult decisions and can work to tight deadlines.
* You will have adult social care experience and good IT skills.
* Hospital Social Care experience is desirable; practitioners should be willing to learn within a fast-paced environment.
* In Warwickshire, we use the Mosaic IT System.
