An interesting and exciting permanent position for band 6 Registered General Nurse (RGN) to join the Continuing Healthcare team in the Central Area covering Conwy and Denbighshire. Fixed term until the end of March 2027. The post holder will manage the implementation, monitoring and review of NHS Funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) in Care Homes (Nursing) and domiciliary settings, ensuring care meets best practice and national standards.
Responsibilities
* The post holder will manage the implementation, monitoring, and review of FNC and CHC in Care Homes and domiciliary settings.
* The post holder will coordinate MDT members to undertake assessments for CHC funding within acute, community hospitals and domiciliary settings, aligning with National Guidance and Local Specifications.
* The post holder will work in partnership with local authorities and the Independent Sector to complete joint reviews for CHC and FNC patients wherever possible.
* The post holder will provide professional nursing advice to staff in Care Homes to ensure appropriate referrals to other clinical team members.
* The post holder will provide and deliver training packages on CHC and FNC.
Requirements & Preferences
* Band 6 RGN with relevant experience.
* Ability to speak Welsh is desirable; English or Welsh speakers are equally welcome.
