Overview
West Kent Primary Care is inviting applications for a PCN Clinical Pharmacist to join the Tunbridge Wells Primary Care Network team. The role offers an opportunity to work as an independent practitioner within a dynamic primary care team delivering high quality patient services. We are looking for a pharmacist who is passionate about developing their skills in General Practice and who can contribute to a culture of excellence and knowledge sharing. Training and development support is available for those without prior experience in this area, provided there is a robust clinical background.
Tunbridge Wells PCN comprises 8 practices serving around 86,000 patients. Pharmacists are based at a chosen practice and collaborate with a wide multi-disciplinary team, including first contact clinicians and care coordination roles.

Main duties
* Develop, enable, and support a culture of multidisciplinary care with a focus on developing PCN services in line with national and local specifications.
* Work directly with practice teams and coordinate with community and acute colleagues to meet patient health outcomes in a collaborative environment.
* Support evening and weekend extended access clinics on a rota with other PCN staff (current setup includes one remote evening per week and one Saturday morning on-site in six).
* Register on the CPPE pathway and Independent Prescribers Course (if not already completed), with time allocated for training and supervision.
* Collaborate with GPs and other healthcare professionals, conducting patient consultations (telephone and face-to-face) and contributing to care plans for long-term condition management (e.g., hypertension, cardiovascular disease, asthma, diabetes).
* Update records, ensure patient safety, manage patients within appropriate care settings, and undertake audits for medication safety.
Qualifications and experience
Essential:
* University Degree in Pharmacy Sciences
* Registered with the General Pharmaceutical Council
* Experience providing clinical advice on medicines to colleagues and patients
* Experience as a community, hospital, or primary care pharmacist
* Experience using MS Outlook and calendar management
* Able to use Microsoft Office (Word, Excel, PowerPoint) proficiently
* Strong communication skills, with ability to explain complex information to varied audiences
* Ability to work independently and as part of a team, managing workload to tight timescales
Desirable:
* Registered Prescriber
* Experience managing long-term conditions in a clinical setting
* Knowledge of structured medication reviews and post-discharge medicines management
* Knowledge of NHS Long Term Plan and medicines optimisation
* Car driver
