Our Social Workers work alongside people and their families and carers to help them think through what might help them get on with their lives in the way that works best for them. We do this within the framework of our statutory Care Act requirements. This means many of our conversations are framed as ‘assessments’, ‘reviews’, ‘support planning’ and ‘safeguarding’. But our aim is always to listen and understand what matters to the person and then to work with them to think through how this might be achieved.
Our Team
What we do :-
1. Provision of statutory Social Care and Social Work Services for Adults age 18 + who have been discharged from an Acute or rehabilitative health provision on a pathway 1 (assessment within the adult’s home environment to determine their long term needs after discharge from hospital) across Essex and the neighbouring acute hospitals. This will be known as the Home to Assess Service.
2. Provision of Social care Occupational Therapy Services, in conjunction with the OT Hub.
3. Care Act assessments outside of the Acute hospitals after the adult(s) have been discharged back home for within their discharge to assess period.
4. Liaise with the Safeguarding team regarding incoming Safeguards and attend the PAH Safeguarding Scrutiny Panel meetings.
5. We take a person centred strengths based approach to delivery that is embedded within the Social Model of Disability. Recovery and prevention is at the heart of our practice.
6. Apply a strengths based approach to all referrals.

If you’re a Social Worker registered with Social Work England and have 3 years post qualifying experience, with that winning combination of creative thought, reliability and practical ability to get things done, we’d really love to hear from you!
You’ll have a broad understanding of our statutory responsibilities and a commitment to meeting these in a person-centred way.
You may have experience of chairing meetings, and you’ll have an ability to develop strong relationships with partner agencies and the local community to provide a supportive framework.
Our Social Worker roles involve travelling around Essex - Mid covers Braintree, Chelmsford and Maldon. A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.
