Join Our Team as a Community Palliative Care Specialist Nurse! We are based at Unit 16 Building Research Establishment, Watford.
Our role is part of a Specialist Palliative Care Service within the Central London Community Healthcare NHS Trust – Hertfordshire Division.

Main duties of the job
* Qualified with experience in palliative / end-of-life care.
* Organised and flexible, able to adapt to the needs of patients.
* Compassionate towards patients and their families.
* Collaborative, with experience working in a multidisciplinary team.
* Proficient in IT, with experience using SystmOne.
* Excellent communication, with strong community nursing skills.
* Skilled in triaging to ensure the best outcomes for patients.
* Holder of a full, valid UK driving licence and access to a car for business purposes (unless exempt under the Equality Act 2010).
Person specification
Education / Qualification
* Registered Nurse with current UK NMC registration.
* First level degree or undertaking a first level degree.
* Teaching qualification or willingness to undertake one.
* Advanced Communication Course completion.
* Do Not Attempt Cardiopulmonary Resuscitation competencies.
* Willingness to undertake further academic or practical study at a higher level (e.g., Master’s degree).
* Extended Nurse Prescribing Qualification.
Experience
* Attained appropriate level of experience to fulfil competencies required in palliative care or oncology.
* Post registration experience working at a senior level in palliative or end of life care.
* Community nursing experience.
* Evidence of experience in a teaching and mentoring role.
* Experience of cross‑boundary working in partnership and collaboration with health, social and voluntary sectors.
* Knowledge of NMC code of professional conduct.
* Understanding of current political and policy context in Primary health Care.
* Knowledge of research processes.
* Knowledge of clinical governance and its implications.
* Knowledge of resource management.
* Up to date evidence of continued professional development and reflective practice in a professional portfolio.
* Proven leadership skills in service development.
* Experience of participation in research, audit and evaluation.
* Experience of leading change and innovation.
* Experience of using counselling skills.
Skills & Knowledge
* Ability to utilise a range of expert communication methods appropriate to situation.
* Use communication theories to inform and organise care packages as appropriate.
* Communicate complex and sensitive information with empathy and reassurance regarding palliative care patients and their families and professionals.
* Use influencing, negotiating and counselling skills in a highly emotive atmosphere.
* Challenge and be challenged regarding triage outcomes.
* Willingness to uphold the Trust’s values.
