Main duties of the job
* Assisting patients with hygiene needs, including care of skin, mouth, eyes, hair, nails, feet and teeth; encouraging self-care when possible.
* Supporting patients with elimination needs, assisting use of toilet facilities or provision of bed pans, urinals, commodes and vomit bowls; caring for urinary catheters, stomas and other aids that enable the patient to perform elimination and recording actions on the appropriate documentation.
* Promoting continence and managing incontinence according to the patient's individual assessment and plan of care.
* Supporting the multidisciplinary team in the prevention and management of damage to patients' skin, ensuring correct moving and handling techniques and use of pressure-relieving aids are used; reporting any changes in skin condition to the registered nurse.
* Informing the registered nurse of any concerns regarding the patient so that they can assess the patient timely.
* Disposing of bodily fluids, soiled linen, spillages etc. in accordance with Trust policy and guidelines.
* Assisting patients with eating and drinking: appropriate meal ordering and provision of drinks, meals and supplements; providing aids as required, assisting in feeding and hydrating those who are unable to perform these tasks themselves, recording nutritional and hydration intake on the appropriate documentation.

Person Specification
Experience and Knowledge
* Understanding of patient-centered care and the role of the HCSW.
* Experience working in a healthcare setting.
Qualifications
* Numeracy skills to Functional Skills Level 1 (GCSE Grade 2-3, D-F or equivalent).
* Basic IT skills, able to use applications such as email on a computer or smart device (e.g., tablet).
* Demonstrable effective written and verbal communication skills.
* Completion of the Skills for Health Care Certificate.
