The Role Support Worker Liberty Centre is currently recruiting Support Workers and Driver Support Workers to join our growing team in Dagenham. As a Support Worker, you will play a vital role in enabling individuals to live fulfilling, independent, and meaningful lives. Key Responsibilities Deliver high-quality, person-centred care tailored to individual needs Support individuals with learning disabilities and Autism Promote independence, dignity, and choice at all times Assist with decision-making and lifestyle choices Administer medication in line with training, policy, and support plans Support service users to access community facilities and activities Encourage social inclusion and community participation Assist with person-centred planning and goal setting Act as a Key Worker or Co-Key Worker where required Support Health Action Plans and Person Centred Plans Attend and contribute to reviews and multi-disciplinary meetings Promote healthy lifestyles and wellbeing Build positive relationships with families, carers, and professionals Work closely with colleagues, Service Coordinators, and Managers Follow all Liberty Centre policies including safeguarding and whistleblowing About You Previous experience in support work or care is desirable Minimum RQF Level 2 qualification (or equivalent) preferred Strong written and verbal communication skills Caring, patient, and reliable approach Ability to work both independently and as part of a team Passion for supporting people to live independently Full training will be provided for the right candidates.
