Social Worker to work within Care Point Hospital. Social Work Team is a dynamic, multidisciplinary group committed to supporting safe and timely hospital discharges. We work closely with health professionals, patients, and families to ensure that care planning is person‑centred and promotes independence wherever possible.
Care Point Hospital Social Work Team supports safe and timely hospital discharges by coordinating person‑centred care plans that promote independence and recovery. Our work reduces delayed transfers of care, improves patient outcomes, and strengthens collaboration across health and social care systems—benefiting both the borough and the wider authority by easing system pressures and ensuring efficient use of resources.
The core responsibility of the Social Worker is to carry out timely, strengths‑based assessments and coordinate safe discharge plans for patients leaving hospital care. The role aims to reduce delayed transfers of care, ensure legal and safeguarding duties are met, and contribute to improved outcomes through collaborative working.
About the Successful Candidate
The successful candidate will be responsible for completing mental capacity assessments, safeguarding enquiries, and contributing to multidisciplinary team discussions. They will need to demonstrate strong communication, decision‑making, and organisational skills to ensure effective care planning and advocacy for vulnerable individuals.
The successful candidate must hold a recognised social work qualification and be registered with Social Work England, with a solid understanding of legislation including the Care Act, Mental Capacity Act, and safeguarding procedures. While experience in hospital discharge planning and multi‑agency working is desirable, support and development will be provided to build confidence in these areas if needed.
