We are looking for a proactive and compassionate Health and Social Care Coordinator to play a central role within Adult Community Services in West Kent. The role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
Key Responsibilities
* Act as a key point of contact for patients, carers and professionals, supporting complex health and social care needs.
* Work across a wide range of partners—including Acute Trusts, Primary Care, Social Services and voluntary organisations.
* Support housebound patients living with long‑term conditions to improve their wellbeing and maintain independence.
* Help reduce unnecessary hospital admissions by responding to referrals, offering advice and support, and signposting to other services where appropriate.
* Manage your own caseload, planning outcomes and expectations with patients and carers to enable them to manage their health needs.

Working Pattern
Additional Working Pattern: 22.5 hours / 3x7.5 hours Monday to Friday.
