Bridge and Monkseaton MedicalPractice is innovative, friendly and hard-working practice. We are enthusiastictraining Practices that host GPR and F2 doctors and medical students. We arebased in two locations, Monkseaton and Shiremoor.
We are looking to recruit acaring and enthusiastic Pharmacist to join our busy multi-disciplinary team. Wewould require the successful candidate to be able to work independently for27/37 hours working 8am - 6pm over 3/4 days.

Main duties of the job
* The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them.
* The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
* The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
* The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
About us
We are a 2 site Practice, Monkseaton Medical Centre based in Whitley Bay and Bridge Medical located in Shiremoor. We are led by 4 GP partners. We employ a Management team, 9 salaried GPs, 1 Practice Nurse, 3 Health Care Assistants, 3 Pharmacists and a Pharmacy technician. We have a navigator system that has been developed specifically to the Practices vision and this helps with continuity of patient care. We are very focused on team development holding monthly staff meetings and holding at least 4 Time In Sessions per year. We hold socialised events and enjoy being part of a large work family.

Key responsibilities
* Long Term Condition Management - face to face or telephone consultations with patients on single or multiple medical problems where medicine optimisation is required. Review the on going need for each medicine and review of monitoring needs and an opportunity to support patients with their medication taking, ensuring they get the best use of their medicines.
* Prescribe medication for long term condition management following local and national guidelines, making appropriate recommendations to other clinicians for medicine improvement.
* Undertake clinical medication reviews with patients and alter medications to improve prescribing.
* Facilitate Group Consultations about long term condition medications and develop suitable resources for these.
* Provide patient facing and telephone consultation clinics for those with questions, queries and concerns about their medications
* Provide assistance and answers regarding relevant medicine related queries from GP's, other practice staff and other healthcare teams
* Review the use of medications associated with unplanned hospital admissions and re-admissions through audit and individual patient reviews
* Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge
* Produce and implement a practice repeat prescribing policy ensuring patients have appropriate monitoring tests in place when required
* Undertake clinical audits of prescribing in areas directed by the clinicians, patient complaints, new guidelines and significant event analysis
* Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components. This includes setting pathways for and implementing Service Level Agreements and Locally Enhanced Services
* Implement changes to medicines that result from MHRA alerts, product withdrawl and other local and national guidance.

Qualifications
* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
