<div><h3>Job Overview</h3><p>The post holder will practise as an advanced practitioner within the cardiac rehabilitation team. Responsibilities include assessment, examination, diagnosis and treatment within an agreed scope of practice throughout cardiac rehabilitation and related services. The role aims to deliver excellent care across advanced clinical practice, including independent prescribing, leadership and management, research and education.</p><h3>Main Duties</h3><p>Provide specialist cardiac rehabilitation knowledge, expertise and skills for patients and their families to promote recovery and self‑management. Work autonomously and as part of a multidisciplinary team to ensure a safe and effective pathway for cardiac patients. Lead and supervise junior staff and provide sound evidence‑based clinical decisions. Ability to speak Welsh is desirable, but Welsh and/or English speakers are equally welcome to apply.</p><h3>Working Locations and Travel</h3><p>Deliver cardiac rehabilitation in a variety of clinical and gym‑based settings in Conway and Denbighshire. The role requires travel across the counties.</p><h3>Person Specification – Essential Criteria</h3><ul><li>NMC Registered Nurse.</li><li>Degree level education with willingness to work towards a master’s level (post‑registration).</li><li>Continuous professional development in a professional context.</li><li>Registered independent prescriber (or a willingness to become one).</li><li>Strong understanding of BACPR standards and use of cardiac rehabilitation elements.</li><li>Knowledge of national guidelines such as NICE, ESC, Welsh Government.</li><li>Extensive post‑registration experience in cardiac care, including previous work at Band 6/7.</li><li>Experience coordinating and managing patient care.</li><li>Experience in leadership, audit or research to develop and guide practice.</li><li>Skilled negotiator with advanced interpersonal and communication skills.</li><li>Committed team worker who demonstrates passion for cardiac rehabilitation and patient care.</li></ul><h3>Person Specification – Desirable Criteria</h3><ul><li>Teaching ability.</li><li>Knowledge of early‑care system.</li><li>Recognition of own developmental needs and experience of audit or research activities.</li><li>Interest in driving development in cardiac nursing.</li></ul><h3>Values</h3><p>Supportive teamwork, role modelling, and a passion for cardiac rehabilitation are essential.
