Role Overview
Applicants must have a valid UK driving license and access to a car insured for work purposes. The role involves working primarily in the community to support individuals accessing secondary emergency care services related to alcohol misuse, facilitating integrated care across services, and supporting high-intensity users of the trust.
Main Responsibilities
* Work with high-frequency emergency department attendees and admitted patients related to alcohol misuse.
* Implement care pathways and liaise between community substance misuse services and hospital teams.
* Extend care beyond discharge, re-engaging resistant or hard-to-engage individuals.
Candidate Requirements
Essential Qualifications
* Registered Nurse - NMC
* First degree or working towards in a related area
* GCSEs in English and Maths or equivalent
* Experience with substance misuse clients and working with multiple agencies
* Current driver’s license and access to a car
Desirable Qualifications
* Prescriber qualification, CBT, clinical skills modules, leadership and motivational interviewing training, and experience in health service management.
Experience & Skills
* Independent working experience, working with individuals with alcohol use disorders, and in community settings.
* Excellent communication, organizational, and IT skills.
