Overview
RN Care Coordinator - Pulmonary Services is responsible for day to day management of patient care from initial contact through recurring visits. Ensures patient care services are timely and provided at the appropriate level of care. The primary goal is to support and engage in activities that result in effective patient care and efficient use of resources along the care delivery continuum.
Key Responsibilities
* Manage patient care from initial contact through recurring visits.
* Ensure timely delivery of patient care at the appropriate level of care.
* Support and engage in activities that improve patient care outcomes and resource utilization along the care delivery continuum.
Minimum Education
* Graduate of an accredited school of nursing; BSN preferred.
Minimum Work Experience
* 2 years related clinical RN experience per specialty area.
* Nursing charge or lead experience strongly preferred.
* Experience educating a diverse population on healthcare issues highly desirable.
Required Licenses, Certifications
* Current Vermont RN license.
* BLS Certification through American Heart Association.
* Valid Driver's License (if conducting home visits).
* Department specific Certifications may apply.

Compensation
Pay Range = $36.15 - $61.15
