Nottingham University Hospital NHS Trust seeks applications for asubstantive CESR Fellow in Acute Medicine based at the Queens Medical Centre Campus.Our CESR workforce currently has 5 fellows in the programme and we are looking to increase the team.
The CESR, or Certificate of Eligibility for Specialist Registration, is the route to specialist registration for doctors who have not completed a GMC-approved deanery training programme but who are able to demonstrate that their specialist training, qualifications and experience are equivalent to the requirements for the award of the CCT in the UK.It is a competency-based programme that enables the trainee to provide a portfolio of evidence of their training, qualifications and experience, meeting the requirements of the Acute Medicine CCT curriculum. Successful completion of the CESR process results in entry onto the Specialist Register and doctors with CESR can apply for substantive consultant posts in the UK.
The post is ideal for doctors who wish to pursue a career in Acute Medicine in one of the largest and busiest Acute Medicine Departments in the country. The post affords an excellent training opportunity for an experienced post CT SHO / Clinical Fellow / Registrar who wishes to pursue a career in Acute Medicine outside a deanery based training programme.
A full range of adult patients attend Nottingham University Hospitals NHS Trust and, under the supervision of a consultant, post-holders will be responsible for the management of those patients who are referred to the Acute Medicine department.
The Acute Medicine middle grade rota is largely split into three different roles:
B3 SpR Responsible for the senior review of new admissions, support of junior doctors, undertaking of clinical procedures and having an overall oversight of the take as well as expediting discharges and maintaining patient flow through the department.
AMRA SpR Responsible for the senior review of new GP admissions and Same Day Emergency Care patients. Support fellow junior doctors, undertake any clinical procedures and maintain patient flow through the department.
AMRAT SpR Receive referrals to Acute Medicine from the Emergency Department to ensure that correct pathways are adhered to and patients are seen by the right person, in the right place and at the right time. This role also offers support for advice to our Emergency Department colleagues.
On-call duties cover both campuses and include GIM on-call, stroke thrombolysis, supporting the medical level 1 unit, and Level 1/Level 2 care for respiratory patients.

We have recently been featured as the new home to Channel 4's '24 Hours in A+E' television show. The show is currently being broadcast on Channel 4.
The post holders will work under the clinical direction of the Consultants in the Acute Medicine Department across all clinical areas.

The CESR posts will include out of hours working on the General Internal Medicine registrar rota. Average weekly hours are expected to be circa 46 42 clinical and 4 hours for CPD. Once a postholder is considered to be competent they may be moved to the consultant on call rota.
During rotations to other specialties the post holder may remain on the QMC GIM rota or may work on the rota of the specialty they have rotated into. This will be discussed and agreed before the rotation starts.
Successful applicants will join a happy and enthusiastic multi-disciplinary team including consultant and junior medical staff, nursing staff, ACPs, prescribing pharmacists and Physician Associates. Our Acute Medicine Unit is being ambitiously and innovatively restructured, expanded and modernised both in terms of physical estate and in the way we work. NUH has one of the busiest Acute Medicine takes in the country with approximately 60% of patients being referred from the Emergency Department (ED) and 40% from Primary Care.
Acute Medicine is made up of three clinical areas, all consultant led between 08.00 and 22.00 7 days per week:
Acute Medicine Receiving Area (AMRA) a 20 bedded ambulatory area to treat and assess patients directly from the community, or the Emergency Department.
Acute Medicine Assessment Unit - ward B3 currently a 64 bedded short stay unit providing assessment to patients admitted from AMRA and the Emergency Department
Acute Medicine Level 1 Unit - 12 beds for the initial care of higher acuity medical admissions. The unit has enhanced nurse to patient ratios, monitoring and increased senior support
We also provide an innovative consultant led Telephone Triage, Advice and Navigation service primarily for GPs which has significantly improved Right Place, First time navigation, improved access to alternative ambulatory pathways, reduced unnecessary admissions, delays and length of stay and allowed many patients to be safely managed in the community with advice.
Physicians in our team enjoy a great breadth and depth of clinical experience and a fulfilling and varied clinical portfolio. Our assessment areas are superbly supported by our Medical and other specialty teams and we enjoy a good working relationship with our Emergency Department. Acute Medicine provides dedicated 24 hour in-reach and advice via the AMRAT service.
