An exciting opportunity has arisen for an Advanced Nurse Practitioner to join our friendly and innovative Primary Care Network for up to 37 hours a week under the Additional Role Reimbursement Scheme. The role involves supporting the teams of our four member practices with acute patients, working alongside two current ANPs and a diverse team including Pharmacists, Social Prescribers, and Care Coordinators within our Primary Care team.
Main duties of the job
As a Network Advanced Nurse Practitioner, you will provide expert clinical care across various settings, ideally based at no more than two sites to ensure full integration with our teams.
Key Responsibilities:
1. Deliver comprehensive assessments and clinical management to patients across multiple clinical areas.
2. Collaborate with GPs, allied health professionals, and other clinical staff to ensure effective care pathways.
3. Lead the development and implementation of evidence-based protocols and policies.
4. Provide leadership and mentorship to junior nursing staff and other clinical team members.
5. Ensure all clinical practices comply with national and local guidelines and legislation.
6. Drive innovation and continuous improvement in patient care within the primary care network.
Candidate Profile
You will be a registered Nurse with an advanced practice qualification and significant experience as an ANP, demonstrating clinical expertise and leadership skills. The role requires the ability to work autonomously, with excellent communication, problem-solving, and teamwork skills. It is essential that you have completed or are working towards a Centre for Advancing Practice-accredited MSc Advanced Practice programme, or have completed the Centres ePortfolio route, or are currently completing one of these approved routes to obtain your digital badge.

Poole North Primary Care Network comprises four practices dedicated to achieving excellent health outcomes for our 52,000 patients across seven sites in Canford Heath, Broadstone, Merley, Corfe Mullen, Creekmoor, and Oakdale. (The location for this role is primarily Canford Heath for management purposes, but up to two bases across our other locations may be provided.)
