Social Worker, Community Care, full-time 37 hrs per week, full-time permanent contract

Salary Scale: NHS Band 6 - £39,912 – 48,635
The Skye and Lochalsh Integrated Health & Social Care Team has a vacancy for a Social Worker within its busy Portree and Broadford Offices. The team works closely with nursing and mental health teams to provide a multidisciplinary and integrated service in a beautiful but remote and rural setting.
This is an exciting opportunity for a qualified social worker to work in a community care context with adults.
We are a small, motivated team supporting adults with care needs in Skye and Lochalsh. Good team working skills are essential as we work closely to support each other in facing diverse challenges.
Roles include working with older adults, adults with learning disabilities, autism, and those transitioning from Children’s services.
Duties will include:
* Supporting vulnerable adults and their families and carers
* Working with clients using a person-centred approach
* Working within relevant legislation including the Adult Support and Protection, Adults with Incapacity, and Mental Health Acts
* Collaborating with NHS professionals for multidisciplinary interventions
* Assessment and report writing
* Arranging care packages with client consent to meet assessed needs
* Working within a large, remote, and rural geographical area, which may present challenges

A driving license is essential due to the rural location, and applicants should have access to their own vehicle for work purposes.
Applicants should be registered with the Scottish Social Services Council as a 'Social Worker' and hold a qualification recognized by the SSSC as equivalent to the Diploma in Social Work (DipSW).
