We are recruiting for a full time Advanced Nure Practitioner to join our newly formed Acute Care team. We will also accept part time or job shares. We will consider a recently qualified Advanced Nurse Practitioner if they are right for the role but they will need to have some previous experience of minor illnesses and acute care.
They willprovide assessment and care of acutely presenting patients within thepractice.
They will be able to work autonomously within the scope of theirexperience and qualifications but will have the support and supervision of the doctorswithin the practice. This includes appointments within the surgery as well as onoccasion, visits to patients in their own homes or nursing/residential homeswhere required.
They will recognise andwork within their own competence and professional code of conduct as regulatedby the NMC. In order to work at this level NMC requirements for advanced clinicalpractice must be met.
Main duties of the job
Our Acute Care Team is made up of a GP and Advanced Nurse Practitioners. We are in the process of having a new suite built within the surgery to allow the team to work in close proximity.
The main duties will include assessing and diagnosing patients presenting with acute issues, either face to face, by telephone or any other methods as appropriate.
You will also assist the nursing team with patients who have presented with issues beyond those that they feel competent to manage.

The Grove Medical Centre is a large dispensing practice based in Sherborne with around 13,000 registered patients. We are a friendly, hardworking team who put patient care first, with every department always willing to help each other out to deliver the best possible care. A fullyqualified Advanced Nurse Practitioner will be competent in delivering the following services. For a registered Advanced Nurse Practitioner entering generalpractice it is recognised that they might not yet have all these skills and assuch training will be given as appropriate.
Clinical
Assess, examine, diagnose, and develop managementplans for patients presenting with a new problem. Deliver evidence-based careaccording to national and local guidelines.
Prescribe within the scope of knowledge in accordanceto both practice, NHS Dorset and National guidance.
Produce accurate complete records of patientconsultation using accurate cod to ensure easy and accurate informationretrieval for monitoring and audit processes.
Assist the practice to obtain the level of QoF pointsrequired by acting on highlighted QoF indicators where appropriate.
Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects, and interactions.
Prioritise health problems and intervene appropriatelyto assist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care.
Support patients in the application of the principlesof self-care and healthy living.
Where suitably qualified, to manage the health needsof women presenting for family planning, cervical cytology, or sexual healthconsultation.
Participate in opportunistic vaccination andimmunisation of both adults and children.
Follow agreed clinical protocols with referral to GPsas appropriate.
If required, to assess and manage the needs ofpatients presenting for opportunistic wound care or phlebotomy provision.
Support the nursing team with assessing patients who havepresented with issues beyond those the nurse/HCA feels competent to managewhere it is within the clinical scope of your own competence.
General

Have an awareness of and compliance with all relevantpractice policies/guidelines, such as prescribing, confidentiality, dataprotection, health and safety.

Chaperoning and assisting patients who are beingexamined by another clinician on request.
Maintain general tidiness and cleanliness of theconsultation or treatment room that you are working in.
Participation in administrative systems in the Practice
A duty to advise GP(s) of potential problems orerrors within the range of assigned tasks.
Attend and participate in any Practice meetingswhen required. These may be on days youdo not normally work, however you are expected to attend and will be paidovertime.
On occasion there may be a requirement toundertake home visits
