Job Overview CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! Are you a qualified Physiotherapist wanting to further your rehabilitation, organizational and management skills, working with patients in their own homes? We are seeking an enthusiastic B and 6 Physiotherapist to join our Planned Care team in Watford community. The position offers experience of a mixed caseload of patients with various pathologies; including amputees, neurological / stroke rehabilitation, Parkinson’s disease, falls / balance impairments, orthopaedics, long-term conditions and palliative / end of life care. The aim is to rehabilitate and maximise the patients' potential, to support and enable patients in a range of activities to promote independent living in their home environment. The team is a well-established and covers a large area and population size, the position requires the assessment and treatment of physiotherapy intervention for patients in their own homes. Liaising with patients, family, carers and other MDT staff. This is an excellent opportunity for an physiotherapist working in the acute, bed-based rehab, or social services, who have complete core rotations in acute setting as a band 5, who would like to experience community working. A full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010) Main duties of the job Provide assessment, treatment, and interventions to patients in their own home environment to support them to optimise their functional ability. You are required to be a car driver and have access to a car to use for work to see patients in their own home across the locality. You may be required to transport and provide equipment from our satellite stores to the patient. You will work as an autonomous practitioner, but you will work closely with other physio's, assistants and OT's. The team also has close links with the community nurses, complex care and frailty team, GP's, palliative care specialists, hospices, social care, and other third party sector organisations. Person specification Qualifications Essential criteria Diploma/degree in Physiotherapy Current HCPC registration Evidence of up to date CPD portfolio Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation Member of Professional Body Desirable criteria Member of a Special Interest Group relevant to this post Experience Essential criteria Experience post registration as a Band 5 or band 6 physiotherapist Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory Desirable criteria Experience working with voluntary organisations. Experience in service development projects Skills And Knowledge Essential criteria Skills in the assessment and treatment of adults with complex multi-pathologies Knowledge and understanding of complex medical conditions including multipathologies and falls Knowledge of role of Allied Health Professionals and interdisciplinary team working Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting Ability to problem solve, prioritise and demonstrate clinical reasoning Good communication and report writing skills, in both verbal and written format Basic IT skills – Microsoft Word, Power point, Excel Knowledge and understanding of the role of Social Services and voluntary agencies Desirable criteria Knowledge and understanding of National Service Framework for Older People Knowledge and understanding of NICE and CSP/COT Fallers Guidelines Key attributes Essential Criteria Able to work across the four localities which make up West Hertfordshire Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
