Full Time, 37 hours per week
The Hospital Discharge Team based at North Devon District Hospital is made up of experienced social workers, social care assessors, nurses, paramedics and coordinators, who work alongside hospital staff to work towards achieving the most appropriate discharge options for those we work with.
The role of the social worker involves supporting individuals to explore their options relating to care and support on discharge from hospital. This involves reviewing care and support needs to identify which of the discharge pathways is most appropriate. Social Workers within the team are also involved in Safeguarding Enquiry work, experience of this is essential within this role.
A sound understanding of the Mental Capacity Act is also essential and it would be beneficial for individuals to have experience of completing complex mental capacity assessments.
Once a person has been discharged from hospital the team a have the responsibility for completing Care Act Assessments for individuals to ascertain an individual’s ongoing needs for care and support. As a social worker in hospital discharge you will play a pivotal role in ensuring that the individuals we work with receive the support they need to thrive.
