You will work closely with children and young people aged 14-25, their families, and professionals across education, health, and social care. Your role will include undertaking OT assessments and reviews whilst also considering equipment, adaptation and moving and handling needs as appropriate. The emphasis will be around preparation for adulthood planning. Deliver holistic, person-centred occupational therapy assessments and interventions. Support young people in developing life skills essential for adulthood. Collaborate with adult social care, schools and colleges to ensure coordinated support. Complete preparation for adulthood plans Promote inclusive practice and advocate for young people’s aspirations and goals. HCPC registered Occupational Therapist with experience in either adult or paediatric services. Excellent communication and teamwork skills. Passionate about promoting independence and inclusion.
